    The America’s Choice 500K Plan  plan is a fixed indemnity co-pay plan. The difference with this indemnity plan is that it offers co-pay’s through the PHCS network. There is a $500,000 yearly benefit limit, meaning anything beyond that the client will be responsible for.  You have 10 Dr visits each year per person.
