Life X – VL Plans

 

 

The Life X VL Plan is a major medical plan, within the PHCS or Anthem (BCBS) Networks.

This plan works as a traditional deductible plan with 2 deductible options, $250 or $500, copays for Dr visits (after deductible)  and FREE Telemedicine.

Preventive care is generally covered by this plan, if staying within network.

There is also allergy treatments, pregnancy and diabetic services offered, along with generous hospital coverage.
