Life X – VL Plans

The Life X VL Plan is a limited benefit plan, within the PHCS Network.

This plan offers several deductible options, copays for Dr visits (after deductible) and FREE Telemedicine.

Preventive care is generally covered by this plan, if staying within network.

This is a more limited care plan with maximum allowed visits for most services.  It is more budget friendly option for those healthier individuals and families.
