America’s Choice Health 1M Plan

The America’s Choice Health 1M Plan  plan is a major medical plan, within the PHCS network.

This plan works as a traditional deductible plan with 3 deductible options, $250, $500 & $750, 0% coinsurance after deductible, but there are copays for Dr visits (after deductible)  and FREE Telemedicine.

Preventive care is generally covered by this plan, if staying within network.

There is also allergy treatments, pregnancy and diabetic services offered, along with generous hospital coverage.
