United Tri-Term Plan

Tri-Term Plans are a great solution for those individuals who are looking for a consistent premium for an extended period of time.  Each plan offers a guaranteed rate for 3 years.

The most popular of the Tri-Term-Term plans is the CoPay Select Max Plan.  It offers several deductible options, with the $7500 being the most popular and 70/30 coinsurance and $4500 man out-of-pocket after deductible, premiums change based on deductible you choose.

There is a $50 copay for Dr visits per person, per term for 4 visits.  There is also $75 copay for unlimited urgent care visits.  $50 copay for well exams, 1 per person per term is also available.
